Services

Members Eligibility

  • Holder of the Hong Kong Identity Card issued under the Registration of Persons Ordinance (Cap. 177, the Laws of Hong Kong) or certificate of exemption , except those who obtained their Hong Kong Identity Cards by virtue of a previous permission to land or remain in Hong Kong granted to him except in case that such permission has expired or ceased to be valid; or a child under 11 years of age of a Hong Kong resident and;
  • Those who agree to enrol to Electronic Health Record Sharing System (eHRSS); and
  • Those aged 6 or above, residing or working in Yuen Long District.

Registration Procedure

  • If you would like to be our member, please bring along the following documents to Yuen Long District Health Centre (YLDHC) for registration:

    • Hong Kong Identity Card
    • Address proof under your name; OR
    • Working permit with your name, company name and company address located in the Yuen Long District

  • If you have the proof of the following medical waivers, please bring to YLDHC for verification:

    • Comprehensive Social Security Assistance
    • Old Age Living Allowance (age 65 or above)
    • Residential Care Service Voucher
      (Co-payment Level 0)
    • Medical fee waiver document issued by the Hospital Authority
