Yuen Long District Health Centre

The establishment of the "Yuen Long District Health Centre" (YLDHC) will provide a series of individual and family-based health risk assessments and health education for people residing and working in the District. They can enrol as members of YLDHC which will become their partner in primary healthcare leveraging on different levels of disease prevention. Focusing on prevention and personalized healthcare plans, multi-disciplinary teams aim to empower people in changing unhealthy habits, learning self-management of diseases in order to strengthen their physical conditions. YLDHC will work hand-in-hand with them to build healthy lifestyles.


Scope of Services

Primary healthcare can be divided into three levels of disease prevention:

Primary Prevention

Through the health promotion, consultation, counselling and educational programs, members are encouraged to build healthy lifestyles for preventing chronic diseases. Health promotion programs include

  • Healthy Diets
  • Physical Activity
  • Weight Management
  • Fall Prevention Training
  • Smoking Cessation
  • Alcohol Consumption Control
  • Sleep Hygiene
  • Mental Well-being

FREE health risk factors assessments for all members

Example: The story of Kuen’s family

Ah Kuen registered as a member of the YLDHC and received free health risk factors assessment, and was advised that she has no immediate physical problem. However, she realized that good eating and exercise habits can help to prevent the risk of developing chronic diseases. She therefore enrolled into different exercise and nutrition classes hoping to apply what she learnt to improve healthy lifestyles of family members.

Secondary Prevention

Health risk factors assessment and screening can help in early identification of health-related problems enabling appropriate follow-up or treatment as needed. Programs include

  • Screening for Diabetes Mellitus and Hypertension

FREE health risk factors assessment and screening
for all members

Subsidy will be provided if referral for further treatment
by Allied Health professionals is needed.

Example: The story of Kuen’s family

Ah Kuen is worried about the health status of her middle-aged husband, Ah Ming who had smoked for many years and developed central obesity. After the health risk factors assessment, he was found that he had a high risk in developing Diabetes Mellitus and Hypertension; thus he was referred to a network doctor for further medical examination and subsequently diagnosed to have Pre-diabetes and Hypertension.

The nurse of YLDHC helped to arrange a series of physical exercising and dietary counseling programs conducted by various Allied Health professionals for Ah Ming. He was also taught to learn self-checking of blood pressure and blood glucose level for monitoring and preventing deterioration of his health conditions.

Tertiary Prevention

Slowing down of the deterioration of chronic diseases can be achieved through chronic disease management programs and community rehabilitation services.

  • Implements chronic disease management program for patients with Diabetes mellitus, Hypertension and musculoskeletal disorders (such as low back pain or degenerative knee pain)
  • Provides community rehabilitative training for patients with Stroke, Hip Fracture and Post-acute Myocardial Infarction

Subsidy will be provided if referral for further treatment
by Allied Health professionals is needed.

YLDHC will also receive cases referred by hospitals for rehabilitation.

Ah Kuen's mother-in-law was referred to the YLDHC for rehabilitation after she was discharged from hospital after an episode of stroke. Treatments include physical therapy, occupational therapy, speech therapy, etc. Social workers will also provide emotional support for patients and their families when needed.
