Eligibility of CDCC

  • Hong Kong residents aged 45 or above*

  • No known medical history of diabetes mellitus or hypertension

  • Have enrolled in the eHRSS and registered as a YLDHC member

Membership Registration Process

  • If you wish to become a member, please bring the following documents to our center for registration:

    • Hong Kong Identity Card
    • Proof of residential address in the applicant’s name; or
    • Employment certificate with their name, company name, and company address

  • If the applicant has the following medical fee exemption certificate, please bring along the following documents for verification by the centre staff:

    • Comprehensive Social Security Assistance Scheme
    • Old Age Living Allowance (75 years and above)
    • Elderly Residential Care Service Voucher (Level 0 Persons)
    • Hospital Authority Medical Fee Waiver Certificate
